Entry Checklist

Did you remember to include:

• one polyboxed DVD-R NTSC (Region 1 coded)
• one CD with three images from the film at 300 dpi or better with photo credits
• completed entry form (4 pages)
• $45, $55, or $75 (Canadian or U.S.) entry fee, based on entry date
• press kit and director’s curriculum vitae (CV), if available
• completed Canada Customs Form affixed to the outside of the package if your entry is from outside Canada. The declared value in Canadian dollars should be equivalent to the materials only (approximately $3.00 CDN per DVD and $10.00 CDN per master).
